Health Form

Prior to entering our program, each child must have a physical examination from the past year. State regulations require a health form, which includes a record of current immunizations, to be completed by the child’s physician. This form must be filed in our office within 30 days of enrollment.

Immunizations

The New Hampshire Division of Public Health Services recommended immunizations for children between the ages of 6 weeks and 6 years.

Creative Years requires all children to have the following immunizations:

  • DTaP (4 or more doses)

  • Hep B (3 doses)

  • IPV (3 or more doses)

  • Hib (4 doses)

  • MMR (at least 1 dose)

  • VAR (at least 1 dose)

A child may be conditionally enrolled if they have had at least one dose of each of the required vaccines (based on recommended age) and an appointment for the next age appropriate dose. Documentation from the pediatrician will be needed.

Illness Policy

Children who arrive at school noticeably ill, or with a rash or fever, will not be admitted. Should a child become ill during the day, the parent will be notified. If you know ahead of time that you will not be at home or at work, please provide us with a phone number where you can be reached.

Doctor's Note

Any child who has been hospitalized or who has missed classes with a communicable disease must provide a signed doctor’s note before returning to school.

Medications

In order for us to administer prescription or non-prescription medications, we must have a written authorization form filled out and signed by a parent or guardian. This is in accordance with the New Hampshire Division of Public Health Services and Bureau of Child Care Standards and Licensing. State law also requires that all prescription medications be accompanied with a form completed by your health care provider.

We will not administer any medication that is not in its original container.

Children who require medications that need to be administered on a regular basis are required to follow the same procedures listed above. This form must be updated every six months
