Health & Safety
Each child who has not had a physical examination within the past year must have one on file prior to entering our program.
Each child is required by State regulations to have on file a health form completed by your child’s physician, which includes a record of up-to-date immunizations. If a child’s health form is not up to date within thirty (30) days from enrollment, the child will be excluded from the program.
The New Hampshire Division Of Public Health Services recommended immunizations for children between the ages of six weeks and six years of age can be viewed at the above link. Download PDF (To save to your system, right click on download link, then select “save link as” from the menu.)
The Hepatitis B Vaccine is only required for children born after 1/1/93.
Illness Policy
No child who arrives at school noticeably ill, with a rash, or with a fever will be admitted for that day. Should a child become ill during the day, the parent will be notified. If you are not planning on being at work, please let your child’s teacher or Director know in case of an emergency.
Doctor’s Note
Any child who has been hospitalized or who has been out of school for communicable diseases must provide a signed doctor’s note before returning to school.
Medications
In order for us to administer Prescription or Non-Prescription medications we must have a written authorization form filled out and signed by a parent or guardian in accordance with the New Hampshire Division of Public Health Services and Bureau of Child Care Standards and Licensing. For all Prescription medicine a form must be filled out by your health care provider as required by state law.
We will not administer any medications that are not in its original container.
Children that require maintenance medications to be administered as an on going basis are required to follow the same procedures listed above. This form must be updated every six months.
