Illness Chart

Doctor's Note

Any child who has been hospitalized or who has been out of school for communicable diseases must provide a signed doctor’s note before returning to school.

Medications

In order for us to administer Prescription or Non-Prescription medications we must have a written authorization form filled out and signed by a parent or guardian in accordance with the New Hampshire Division of Public Health Services and Bureau of Child Care Standards and Licensing.  For all Prescription medicine a form must be filled out by your health care provider as required by state law.  A copy of this form is located in the Appendix of this handbook.

*We will not administer any medications that are not in its original container.

Children that require maintenance medications to be administered as an on going basis are required to follow the same procedures listed above.  This form must be updated every six months.

Illnesses and Accidents

Should a child develop an illness during the day parents or guardians will be immediately notified.  In helping us to quickly reach you, please keep your child’s registration form up-to-date with current phone numbers and contacts.

If your child should have a non-serious accident during the day (i.e. scrape, bruise) we will take the necessary steps in caring for these injuries and in comforting the child.  In such cases you will find an accident report in your child’s cubby detailing the accident and the care given to attend to the injury.  We ask that you sign these reports and give them back to the classroom teacher to keep on file.

Safety Policies  & Medical Emergencies

Our staff is informed of safety rules and commonly occurring incidents. They receive detailed instruction on evacuation procedures and how to report an accident. There is at least one staff member on duty at all times who has a current first aid and CPR certificate.

In case of a medical emergency the center will follow guidelines that attend to the immediate need of the child.  The proper authorities will be contacted along with the parents.

Contagious Illness Reporting

Please notify us if your child has been diagnosed with a contagious illness and will not be at school.  All notifications of contagious illness will prompt us to post a note in that child’s room indicating the type of illness and the symptoms.  The identity of any child with a contagious illness will remain confidential.  If at any time you have questions or concerns please do not hesitate to contact us.

Hygiene and Personal Cleanliness

At the beginning of each school year we teach all our children the importance of hygiene and personal cleanliness.  We particularly stress the importance of hand washing after using the bathroom, returning from outdoors, before and after snack and lunch and at any other time that our hands may need to be sanitized.  Such behavior is encouraged and modeled throughout the year as it not only teaches children good hygiene habits, but also reduces the spread of illness.

Prevention

The children are under direct adult supervision at all times.  Fighting is not allowed; running and throwing of objects are not allowed except as a supervised part of the curriculum.  At any time the children leave the fenced in yard as a group they are required to walk in an orderly fashion and are accompanied by the staff.  Parents are required to bring their child into the building, and pick them up inside each day.  Parents are not allowed to take their child from the yard under any circumstance.  All families must enter and leave through the front door.  No child is to exit the building alone.

Sunscreen & Insect Repellent

sunscreen and insect repellentWe ask that all children arrive to school wearing sunscreen from spring through fall.  Parents may provide their child with sun hats or caps as well.  We also ask that if you wish to have sunscreen applied to your child throughout the day you provide a bottle of sunscreen with your child’s name clearly marked on the bottle.  No child will be allowed to share or borrow another child’s sunscreen.  This time of year we also contend with mosquitoes and other pesky bugs.  If you wish to send your child in with insect repellent you may do so.  We ask that you check all sunscreen and insect repellent for expiration dates.  We will not apply any product that has expired.

Evacuation / Fire Drills

Creative Years conforms to all fire regulations including smoke detectors and sprinklers as designated by the State Fire Marshall and the Nashua Fire Department.  Monthly fire drills are conducted and logged by the Director.  In the event of a fire, the building shall be immediately evacuated.  All staff members will remain with the children and see them to the designated areas.
